2009 H1N1 Flu Clinic Schedule
( H1N1) clinic for persons age 18 years and older on Thursday, February 4, 2010 from 10 A.M. to 12 noon at Saint Mark's Parish Center, 467 South Quaker Lane, West Hartford, CT
This clinic is intended for residents of West Hartford and Bloomfield or persons working in West Hartford or Bloomfield. Please complete the required pre-registration information for the WHBHD sponsored Influenza A (H1N1) clinic.Vaccine will be injectable containing thimerosal. The vaccination is free. The vaccine supply is limited. Pre-registration is required. If you need assistance completing the on-line pre-registration please call (860)561-7900 and choose item 4 on the automated voice menu. At the clinic you will be required to complete and sign a consent form.

This clinic is intended for residents or students of West Hartford and Bloomfield or persons working in West Hartford or Bloomfield. Please complete the required pre-registration information for the WHBHD sponsored Influenza A (H1N1) clinic. Vaccine will be injectable containing thimerosal. The vaccination is free.
