Mental Health Case Management
A number of psychiatric, medical, rehabilitative, legal, residential and financial services are available to adults with a history of severe and prolonged mental illness. Case managers work closely with clients to help make their transitions from hospitalization to community life smooth ones, and to help them avoid unnecessary re-hospitalization. While any person meeting the diagnostic criteria is eligible for the program, priority is given to individuals being discharged from psychiatric hospitals.
